From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) Per the pilot's attached statement: "while on a ground run in front of the hangar at idle all systems were working properly without any lights. Suddenly the landing gear light illuminated and the right gear down and locked light turned off. I experienced a jerking in the helicopter so I shutdown both engines simultaneously. The jerking continued so I then shut both master battery switches off. The jerking stopped and as the rotor blades came to a stop the aircraft slowly leaned to the right side and came to a rest on the landing gear door". At the liberty helicopter's request the pilot was sent for a drug and alchol test.
