From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

A Bell 47 helicopter was involved with a hard landing during a FAA flight test with a designated pilot examiner on board. During the execution of a steep approach at the airport the applicant allowed the aircraft to increase in vertical speed. The applicant noticed the increase of vertical descent and increased power as if to start the process of a go-around but instead applied aft cyclic, the aircraft settled to the ground and bounced starting a left yaw. The damage to the aircraft was a bent cross tube. The designated pilot examiner issued a disapproval notice to the applicant.

THE DAMAGE TO THE AIRCRAFT WAS A BENT CROSS TUBE. THE DESIGNATED PILOT EXAMINER ISSUED A DISAPPROVAL NOTICE TO THE APPLICANT.
