From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Following the engine replacement, an engine run (ground) and post run inspection was completed by a line pilot, no discrepancies were noted and the helicopter was released for a maintenance flight. This incident flight was the helicopter's first flight following the completion of the engine replacement; about twenty five minutes of flight, the pilot reported hearing a loud bang, followed by the engine increased on the engine oil pressure up to 160 PSI, and then completes loss of engine oil pressure. A 180 degress autorotation (power on) was performed landed on a dirt road from east to west (latitude 17 58 43.46 north, 6620 09.29) 15.29 miles of ponce airport. Post incident inspection of the engine revealed an engine oil tank exploded and resulted in oil starvation and subsequent engine failure. The mechanics that performed the engine replacement were present during the maintenance flight. The engine s/n 848121 was shipped to standard aero and will be disassembled, standard aero will provide a teardown report and photos at a request of privacy data omitted ( rolls royce customer support regional manager)and soon after to the FAA, FSDO-19 san juan office. A serviced difficult report (SDR) was submitted by the operator (SDR number 2010FA0000134)
