From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

United airlines (uala) flight 950, a Boeing 767, departed iad runway 01R at 21:44 hours (UTC). Uala operations received call that something had fallen from the aircraft. Airport operations performed a check of the runway, finding a pothole in the runway and debris that was believed to be from UAL950. The aircraft returned to iad, landing on runway 30 at 00:38 hours (UTC) with a blown tire. Aircrew maneuvered aircraft to taxiway, where passengers evacuated via people movers. No reported injuries to passengers or crew. The #3 tire was removed and replaced while the aircraft remained in the taxiway, then towed to maintenance area. Inspection revealed that the #3 aft tire had burst on take-off. Additionally, the j-box on the associated landing gear (right main) was misaligned and the #8 spoiler had a small hole. Maintenance performed tire loss and tread loss inspections per aircraft maintenance manual, and in addition to the #3 tire, replaced the #4 tire as a precautionary measure. The j-box was re-alligned to its proper position. The spoiler was temporarily repaired, then deferred for 600 cycles or next heavy check. The aircraft was returned to service and departed for brussels the following day. Iad airport operations revealed that there was a pothole at the beginning of runway 01R, approximately 10' right of centerline. The pothole was a previous patch that became dislodged and had been blown out, as the actual patch was found on the right apron of the runway. Continued inspection of the runway revealed tire debris spanning mid runway, presumed to be from UAL950. The pothole was repaired and the runway was placed back in service.
