From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

The aircraft was on an instruction flight doing a practice ILS approach. Upon landing the crew elected to do a touch and go. The student pilot inadvertantly grabbed the gear handle and selected it to the up posistion thinking it was the flaps. The instructor noticed the student doing so but could not respond quick enough before the gear hanle was selected in the up position, resulting in the nose gear and the right main gear collapsing. The aircraft departed the right hand side of the runway. The weather was VFR and no injuries were reported. Corrective action: school is changing policy to prohibit touch and go takeoff and landings.
