From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

The purpose of the flight was for the staff CFI to check out a new member pilot of the organization (pilots for christ - a non-profit charity). From 3,000' MSL over klry, the CFI (PIC) initiated a simulated engine failure. The pilot flying entered into a spiral over the airport, and attempted to extend the landing gear prior to turning base leg (actuated through the use of a "johnson bar"). At touchdown, the acft bounced, and on second contact with the runway, the landing gear collapsed. Post-incident investigation found no mechanical irregularities with gear system. Neither pilot could recall that the johnson bar had "clicked" into place. That would have ensured a positive locked-down position. CFI stated that, although he reminded pf to extend landing gear, he did not observe a green gear down light.

As the FAA filed it
