What happened
The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.
After flight 1354 landed, without the flight crew's knowledge, the right hand outboard brake failed and overheated. The heat caused ignition of residue and paint on the brake assembly resulting in a fire. The crew, unaware of the presence of a fire, taxied to gate 51D. At the gate, ground service personnel observed the fire and signaled its presence to the flight crew. The captain initiated an evacuation as gate personnel extinguished the fire. A successful emergency evacuation was conducted in less than two minutes using both forward door exits. There was no break in the evacuation action as the ground crew assisted in the evacuation. There were no malfunctions of the components and equipment used in the evacuation. One passenger had a minor injury, an abrasion from contact with the door opening, and declined treatment. The right hand outboard brake had damaged carbon rotors and lost a piston. Personnel recovered sections of the carbon rotors and parts of the piston from the taxiway and runway exit area. The aircraft manufacturer recommended conducting a high heat inspection. There were no findings from the inspection. The brake and the recovered pieces were shipped to bf goodrich for evaluation. When the results of the evaluation are released, the information will be included in an ammendment to this report. Radio communication protocols were not followed during this incident. There were two chautauqua airlines flights (1354 and 1458) and the FAA ground controller (FAA ground) on the ground control frequency before and during the fire and evacuation. The crew of flight 1354, which had the fire, did not use its call sign at any time they communicated with FAA ground. One time the crew of 1354 acknowledged to the FAA controller by keying the mike. The crew of flight 1458, which arrived one minute after flight 1354 and notified FAA ground of the fire on flight 1354, used its ATC call sign twice in the eight transmissions to ATC. FAA ground used complete ATC call signs 5 times in the 10 transmissions to the chautauqua flights. A transcript of this communication follows:grd: "chautauqua 1354, milwakuee ground, accept that was you, turn right on alpha, taxi echo, mike, and bravo back to the ramp"1354: "right on alpha, echo, mike, bravo, back to the ramp"grd: "chautauqua 1354, it appears your left engine is smoking pretty good." 1354: "ok thanks"1354: "was it just smoke"grd: "yeah what I can tell, it is what it looked like, it just puffed out a little bit, about 30 yards behind you" 1354: "ok thanks"grd: "it looks like it quit for the most part"sound of several keyed mike CLICKS1458 female voice: "ground, chautauqua 1458 is going to....."grd:"chautauqua 1358, ah, we just got a call it looks like it could be a brakes or a tire or something, where this might be where the smoke is coming from, um, possible fire, so you better check that out real quick"1354: "roger"1458 female voice (f): "evacuating"grd: "chauauqua 1458 what's the gate again?"1458(f): "evacuate ok, where we are at. Right now we are going to evacuate"1458 male voice (m): "you telling me there is smoke coming out of my airplane?"grd: "not for 1458, no"1458(m): "oh, OK. You almost had us evacuate mam"grd: "oh, OK. As the FAA filed it
AFTER FLIGHT 1354 LANDED, WITHOUT THE FLIGHT CREW'S KNOWLEDGE, THE RIGHT HAND OUTBOARD BRAKE FAILED AND OVERHEATED. ONE PASSENGER HAD A MINOR INJURY, AN ABRASION FROM CONTACT WITH THE DOOR OPENING, AND DECLINED TREATMENT.THE RIGHT HAND OUTBOARD BRAKE HAD DAMAGED CARBON ROTORS AND LOST A PISTON. WHEN THE RESULTS OF THE EVALUATION ARE RELEASED, THE INFORMATION WILL BE INCLUDED IN AN AMMENDMENT TO THIS REPORT.RADIO COMMUNICATION PROTOCOLS WERE NOT FOLLOWED DURING THIS INCIDENT. I'AM SORRY, STEPPED ON TRANSMISSIONS, IT WAS YOUR COMPANY THAT IS THERE, PULLING INTO THE GATE, BUT NO, YOUR'E FINE YOU ARE GOOD TO GO AND WHAT IS YOUR GATE AGAIN SIR?"1458(F): "54G"GRD: "CHAUTUAQUA 1458 ROGER, TAXI VIA, AH, TAXI VIA MIKE AND THEN HOLD YOUR POSITION, RIGHT THERE ON BRAVO, AND MAKE SURE YOUR COMPANY IS INTO THE GATE, AND THEN YOU CAN TAXI TO THE GATE BRAVO INTO THE RAMP."1458(M): "MAM, THEY ARE IN THE GATE, THEIR RIGHT MAIN IS ON FIRE, GET THE FIELD TRUCKS OVER, UM, GET THE FIRE TRUCKS OVER HERE".OPERATIONS: "GROUND OP TWO CONSIDER THE AIRPORT CLOSED, ALERT THREE"GRD: "OPS TWO ROGER"1458(M): "GROUND, CHAUTAUQUA 1458 IS STOPPING HERE"GRD: "ROGER, THANK YOU"
