From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

N892PH a SK-92 helcopter departed Galveston at 0651 for and IFR flight to AC857 with 17 passengers and 3 crew members. At approximately 0745 the crew transmitted a call to phi communicatons center and a mayday call o houston center that they had lost hydraulic pressure to the #2 system and were making an emergency desent to a platform EB110. It was discovered that the #2 heat exchanger failed due a rupture caused by over-pressurization. Inspection determined the quick disconnect coupling associated with the #2 hdraulic pump return line quick isconnect was approximately 1/2 turn from beng completely seated. Sikorsky aircraft determined the orfice of the quick disconnect coupling could possibly be restricted by 28% at 1/2 turnfrom the seated position which could lead to an over-pressurization situation.

As the FAA filed it
