From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

At approximately 13:13 PST inspectors cotry shearrill and britt boutin were called to investigate an aircraft that had gone off runway 16R at Van Nuys Airport kvny. Upon arrival at the scene the aircraft was resting in the center apron between runway 16R and taxiway a about 25 yards south of intersection c. the flight crew was identified and interviewed. The pilot in command was identified as privacy data omitted. Mexican transport pilot privacy data omitted. He stated that the other occupants were his second in command and a technician seated in the jump seat. privacy data omitted Stated that they had just gotten the aircraft out of maintenance, had done a normal pre flight inspection and fueled it to 17,400 lbs. He went on to say that he had calculated the weight and balance but had not written down the location of the CG. Their take off weight was calculated to be 25,800LBS and recorded on the told card. They had calculated a balanced field length within the limits of the available runway. This was not written down either. V1, V2 and vr were calculated to be 128KIAS, 127 and 130 respectively. These were also recorded on the told card. Take off thrust and climb thrust settings were calculated and recorded on the told card as well (1.53 and 1.45 respectively). privacy data omitted Went on to state that the aircraft was taxied out to runway 16R and all pre take off checks were completed. privacy data omitted Was asked how the aircraft was configured for take off. He said that they had set the flaps to 15 degrees (take off setting) and all three trims were set indicating in the green arc. As they reached the approach end of 16R tower asked them if they could accept an immediate take off clearance which they accepted. He went on to say that take off thrust was set and the aircraft accelerated normally. The co pilot called out V1 and vr speeds at which point privacy data omitted attempted to rotate but noted that the nose of the aircraft did not leave the ground. He made a second attempt pulling full aft on the yoke and again the nose would not lift. He then aborted the take off by reducing power to idle, applying full wheel brakes and deploying speed brakes. His first officer also applied brakes on his side. He said he noticed that they where about to run out of runway so he also deployed a drag parachute from the tail cone. He said that it felt like they blew a tire on the left side because the aircraft veered left abruptly exiting the runway and coming to rest in the dirt. They then shut the aircraft down and egressed. An inspection of the aircraft indicated that all four main wheels had deflated. Both left main tires had been torn off of the wheels. The left main landing gear was buried approximately 1 foot in the dirt making it impossible to inspect the wheels. The horizontal stabilizer trim was noted to be in the full nose down position as indicated by reference marks on the vertical stabilizer. An inspection of the cockpit confirmed the trim positions. The pitch trim was indicating full nose down with the rudder and aileron trims indicating in the green arc. The flaps were set to 0 "full up". It was also noted that emergency pitch trim had been activated. Activating emergency pitch trim locks out normal pitch trim functions on the pilots yoke. The only way to set pitch trim in this configuration is by moving the emergency trim knob for or aft. The fuel load was noted at 17,710 lbs total.

As the FAA filed it

THE PILOT IN COMMAND WAS IDENTIFIED AS ^PRIVACY DATA OMITTED^. MEXICAN TRANSPORT PILOT ^PRIVACY DATA OMITTED^. HE STATED THAT THE OTHER OCCUPANTS WERE HIS SECOND IN COMMAND AND A TECHNICIAN SEATED IN THE JUMP SEAT. ^PRIVACY DATA OMITTED^ STATED THAT THEY HAD JUST GOTTEN THE AIRCRAFT OUT OF MAINTENANCE, HAD DONE A NORMAL PRE FLIGHT INSPECTION AND FUELED IT TO 17,400 LBS. ^PRIVACY DATA OMITTED^ WENT ON TO STATE THAT THE AIRCRAFT WAS TAXIED OUT TO RUNWAY 16R AND ALL PRE TAKE OFF CHECKS WERE COMPLETED. ^PRIVACY DATA OMITTED^ WAS ASKED HOW THE AIRCRAFT WAS CONFIGURED FOR TAKE OFF. THE CO PILOT CALLED OUT V1 AND VR SPEEDS AT WHICH POINT ^PRIVACY DATA OMITTED^ ATTEMPTED TO ROTATE BUT NOTED THAT THE NOSE OF THE AIRCRAFT DID NOT LEAVE THE GROUND. THEY THEN SHUT THE AIRCRAFT DOWN AND EGRESSED.AN INSPECTION OF THE AIRCRAFT INDICATED THAT ALL FOUR MAIN WHEELS HAD DEFLATED.
