From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Just after rotation at approximately 45 knots and 25 feet altitude aircraft pitched left. Pilot applied hard right rudder, aircraft bounced, went back in the air several feet, came down and left wing struck the ground. Pilot applied right aileron, aircraft rolled right while contacting the ground, pilot applied brakes causing nose to dip resulting in prop strike with right wing striking the ground. Aircraft skidded to a stop. Ground scars indicate a distance of approximately 150' between first ground strike and stop.
