From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

privacy data omitted Was instructing privacy data omitted during the takeoff departure roll. privacy data omitted Was at the controls during the attempted takeoff (left seat). As the aircraft was proceeding down turf runway 20 at approximately 25 mph, privacy data omitted instructed privacy data omitted to push forward on the controls to bring the aircraft to a level attitude (raise the tailwheel off the ground). privacy data omitted Pushed the left controls forward, but privacy data omitted felt that the aircraft was still nose high attitude and pushed the controls further forward from the right controls (without privacy data omitted knowledge) and the aircraft came to a level attitude. At approximately 35 mph the propeller contacted the ground, nosed over, and the aircraft became inverted. This occurred 110 yards down the runway.
