From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

On 24 August 2011 privacy data omitted contacted planesmart director of client services, MS privacy data omitted, requesting the use of N227TX on October 25-26. Email communications indicated the aircraft was available. In an email from privacy data omitted at 1050AM the proposed flight plan route was provided and he requested "tabs or better on fuel". His proposed departure time was 0700 returning to Addison about 6PM. On October 24, 2001 privacy data omitted amended his departure time from 0700AM to 1000AM. At approximately 1000AM privacy data omitted arrived at the planesmart facility located at 15841 Addison rd, Addison, TX. It cannot be confirmed by planesmart personnel that a preflight of N227TX was completed by privacy data omitted. At approximately 1058 am N227TX departed Addison Airport in IFR conditions bound for houston hooks airport (kdwh) N227TX was advised of radar contact by dfw TRACON and eventually received instructions to fly heading 180. At approx 1107AM ATC radar data indicates N227TX began a left turn. At approximately 1108AM privacy data omitted reported to ATC of a rough running engine and requested assistance direct back to Addison. ATC issued N227TX a clearance back to Addison and also asked if he wanted to declare an emergency. privacy data omitted Response was: "no not at this time". At approximately 1109 am ATC again queried N227TX as to the need to declare an emergency. Again privacy data omitted refused the request for assistance. Shortly thereafter ATC asked N227TX if he was going to need ground assistance upon arrival at Addison. privacy data omitted Indicated no assistance would be needed. N227TX arrived at Addison Airport and was cleared for the ILS to rwy 15. TRACON dallas north controller cleared N227TX for the ILS rwy 15 at 1121AM. According to ATC radar data N227TX was established on the localizer outside jerit, initial approach fix at 1123AM. Shortly after passing jerit, Addison tower radar data indicates N227TX began going right of course. The Addison tower personnel observed this deviation and also noted N227TX'S altitude was below the glideslope. Addison tower personnel notified N227TX of his low altitude and off course situation. At that point N227TX initiated a missed approach. Addison tower issued a clearance to climb to 2000 ft and turn left heading 050. N227TX was then passed back to dallas north controller for vectors back to the rwy 15 ILS for a second attempt. At 1131AM N227TX was cleared by dallas north controller for the ILS rwy 15. Within seconds of turning on course N227TX stated he was not receiving the localizer, and asked if it was working. At 1132 N227TX declared an emergency. ATC issued a heading of 150 degrees, and 4 miles from the airport. At 1133:19AM radar data indicated N227TX began a left turn. At 1133:54 the radar data tag for N227TX was lost from the radar screen.FAA investigators arrived on scene at approximately 1245PM. As a result of the crash sequence the right wing was detached from the aircraft. There was no odor of fuel anywhere. The left wing was still attached to the aircraft and vertical to the ground. There was no fuel noticed to be leaking from either wing. When the Continental motors, inc air safety investigator inspected the engine there were no signs of fuel. The pilot and front seat passenger survived the crash. The passenger in the back seat did not. Numerous attempts have been made by the investingating inspector to obtain information to complete blocks 29 and 31. However pilot has not responded to any requests.
