From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

On 11/17/2011 at about 0100Z, about 3 – 4 miles southeast of kjan, an EC-135 (N389PH) EMS helicopter struck 2 or more birds. The EC-135 was traveling about 90 KIAS at approximately 0900 feet AGL when one or more birds struck the top of the right front canopy in front of the pilot. The right canopy was completely broken out. Bird debris was concentrated on the top left center post and in the rear of the center consol in the flight deck. The pilot sustained minor injuries to his face and left arm. No other injuries were reported. Damage to the aircraft is the right front canopy and there were some small gouges in two of the main rotor blades. There were 5 souls on board the EMS helicopter. The EMS helicopter was transporting a burn patient from M24, griffen/wiggens ms to the crossgates burn center in brandon Ms. The EMS helicopter was crewed by the pilot and two EMT personnel. The burn patient's wife was riding as a passenger in the front left seat. The pilot stated he was starting a decent into the burn center and had just raised his visor to lower his goggles to check for obstacles when the birds hit the canopy and came through into the flight deck. He realized that he had flown past the hospital and saw the kjan airport about 2-3 miles away. He declared an emergency and landed the aircraft on the east side of the airport at taxiway A3.
