On Sunday , January 1, 2012, at approximately 1437 local time parachutist (teresa elaine mclaughlin-wrage) impacted the terrain at Zephyrhills Municipal Airport (kzph) Zephyrhills, Florida.1.name: teresa elaine MCLAUGHLIN2.experience: 600 to 700 jumps 3.club affiliation: parachutist over 40 CLUB4.the harness/container system was a sun path products, inc. javelin tjn sport parachute harness/container system serial #21372 approved under TSO C23C, category b. manufactered, September 2001 in Zephyrhills, FL.the reserve parachute was a performance designs, inc. PD-126-R, manufactered in April 2001. It was assembled correctly onto harness/container. The reserve container had a cypress expert automatic activation device (aad) model #E/1/F, serial # 2081D665CC822 10 manufactered July, 2001 that had been retested on 30 November 2009 and was certified airworthy thru: 10/20/2013 with the last battery change: 03/2009. The unit had not been fired as the reserve closing loop was not cut. The reserve parachute had been repacked by rigger # privacy data omitted whose seal is sxt on 22 October 2011 in Zephyrhills, FL at which time it was noted on packing data card a-i-r "new batts"the main parachute was a pd spectre 120
