What happened
The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.
On January 15,2012 at approx. 1400 the crew of this SW-4 Agusta were conducting icing certification checks which involved hovering sideways while temporarily switching off the hydraulic boost. During the left sideways hover test, the helicopters cyclic control became increasingly harder to control to the point of having to apply as much as 60# of pressure. The helicopter slowly rolled over settleing onto the runway on its left side. These pressures were not noted during the right sideways maneuver. An accumulator or hydraulic pump failure is suspectthe helicopter was shipped back to the Agusta manufacturer for teardown and inspection.
As the FAA filed it
ON JANUARY 15,2012 AT APPROX.
