From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Aircraft was dispatched on a hospital mission. Aircraft was at the airport, kssf, at the time. Start was normal, runup was normal, and t/o to a hover was normal. Aircraft systems checked normal, as did flight control response check and c.g. check. Aircraft received t/o clearance and right turn out on course. As the aircraft started in forward flight, pilot felt a slight vibration in the pedals as the aircraft went through etl. As the airspeed increased to approximately 65 KIAS, the pilot felt a significant vibration in the pedals and the airframe, the crew in the rear of the aircraft stated that they heard a loud rumbling sound and the vibration caused medical gear to fall out of place. Mission was aborted, aircraft returned to land at the airport and maintenance was notified. Inspection revealed a 1 inch gash on one blade of the tailrotor assembly.
