What happened
The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.
Aircraft N566BN departed runway 4R from ffz. At the approximate time of departure the weather was reported as temperature of 73 degrees, winds from the 020 degree heading at 5 knots, visibility at 40 statute miles and few clouds at 15000. At approximately 400 ft AGL and 1100 ft. from the end of the runway, on the upwind departure, the aircraft descended into terrain near an earth wall on the east end of an open field. Immediately following the impact there was a post crash fire which completely destroyed the aircraft. An eyewitness statement taken from a person located five hundred feet from the scene of the accident indicates that he witnessed the aircraft fly approximately two hundred feet over him while he was stopped at the light. He did not hear any abnormal sounds, such as roughness, coming from the aircraft. The aircraft flew over the point of impact and he saw the aircraft begin a steep bank to the right with a nose down pitch attitude and make one and a half turns until it impacted the terrain. He then witnessed the aircraft immediately catch on fire and that the fire was intense. The aircraft was discovered to be facing approximately a southwest heading, one hundred and eighty degrees opposite of its departure, a northeast heading. There was a strong smell of automotive gasoline present at the scene. Pieces of a three bladed warp drive propellor were found in three different locations. Two pieces were approximately 250 feet from the site of impact and one was located on an earthen wall directly behind the point of impact approximately 10 feet up. All three pieces of the propellor were broken off approximately 6 inches from the propellor hub. All three pieces showed scoring and rotational impact marks on the outer edge of the blade near the tips.a turn coordinator found at the accident displayed the aircraft to be in a steep non-standard turn to the right at the time of impact. A safety parachute for the aircraft was located south of the accident site, laying on the ground still partially folded. The pull handle for the parachute was discovered in the wreckage and the safety ring was still installed. Several control linkages were identified in the wreakage a later inspection of the airframe found there to be proper continuity of control linkages. The aircraft flaps were disintegrated from the fire but there was an outline of the flaps on the ground at the site. The flap handle actuator was found in the wreckage but a flap position could not be determined.a later inspection of the engine from a Rotax representative found there was no indication that the engine was operating abnormally at the time of the accident. The Rotax representative tore down the engine inspecting the oil system, the fuel system, the cooling system, the air induction system , exhaust system and the ingnition system and found no anomolies. In addition the representative also inspected the warp drive propellor system and found no anomolies.
As the FAA filed it
AIRCRAFT N566BN DEPARTED RUNWAY 4R FROM FFZ.AT THE APPROXIMATE TIME OF DEPARTURE THE WEATHER WAS REPORTED AS TEMPERATURE OF 73 DEGREES, WINDS FROM THE 020 DEGREE HEADING AT 5 KNOTS, VISIBILITY AT 40 STATUTE MILES AND FEW CLOUDS AT 15000. HE THEN WITNESSED THE AIRCRAFT IMMEDIATELY CATCH ON FIRE AND THAT THE FIRE WAS INTENSE.THE AIRCRAFT WAS DISCOVERED TO BE FACING APPROXIMATELY A SOUTHWEST HEADING, ONE HUNDRED AND EIGHTY DEGREES OPPOSITE OF ITS DEPARTURE, A NORTHEAST HEADING. THERE WAS A STRONG SMELL OF AUTOMOTIVE GASOLINE PRESENT AT THE SCENE.PIECES OF A THREE BLADED WARP DRIVE PROPELLOR WERE FOUND IN THREE DIFFERENT LOCATIONS. TWO PIECES WERE APPROXIMATELY 250 FEET FROM THE SITE OF IMPACT AND ONE WAS LOCATED ON AN EARTHEN WALL DIRECTLY BEHIND THE POINT OF IMPACT APPROXIMATELY 10 FEET UP.ALL THREE PIECES OF THE PROPELLOR WERE BROKEN OFF APPROXIMATELY 6 INCHES FROM THE PROPELLOR HUB. THE PULL HANDLE FOR THE PARACHUTE WAS DISCOVERED IN THE WRECKAGE AND THE SAFETY RING WAS STILL INSTALLED.SEVERAL CONTROL LINKAGES WERE IDENTIFIED IN THE WREAKAGE A LATER INSPECTION OF THE AIRFRAME FOUND THERE TO BE PROPER CONTINUITY OF CONTROL LINKAGES.THE AIRCRAFT FLAPS WERE DISINTEGRATED FROM THE FIRE BUT THERE WAS AN OUTLINE OF THE FLAPS ON THE GROUND AT THE SITE.THE FLAP HANDLE ACTUATOR WAS FOUND IN THE WRECKAGE BUT A FLAP POSITION COULD NOT BE DETERMINED.A LATER INSPECTION OF THE ENGINE FROM A ROTAX REPRESENTATIVE FOUND THERE WAS NO INDICATION THAT THE ENGINE WAS OPERATING ABNORMALLY AT THE TIME OF THE ACCIDENT.THE ROTAX REPRESENTATIVE TORE DOWN THE ENGINE INSPECTING THE OIL SYSTEM, THE FUEL SYSTEM, THE COOLING SYSTEM, THE AIR INDUCTION SYSTEM , EXHAUST SYSTEM AND THE INGNITION SYSTEM AND FOUND NO ANOMOLIES. IN ADDITION THE REPRESENTATIVE ALSO INSPECTED THE WARP DRIVE PROPELLOR SYSTEM AND FOUND NO ANOMOLIES.
