From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Gso FSDO received an anonymous call on June 25, 2012 reporting that a kolb Firestar ii had crashed on take off Saturday June 23, 2012 about mid morning. The pilot was reported as privacy data omitted and the owner as privacy data omitted. The aircraft crashed into trees just off of the runway on the left. It was white and yellow and had been removed from the trees. Tuesday June 26, 2012 two inspectors from this office drove to Elkin airport. They found a crash site which had many trees cut/removed and found a piece of a composite propeller blade/hub assy. Investigation found the pilot at his residence and an interview was conducted there. The pilot stated he was unsure if he had to report anything as he thought it was an ultralight. He stated his intent was initially just to taxi, but the aircraft picked up speed quickly so he decided to put more power to it and take-off. At about 75-100 feet he reduced the power as he thought he might be overspeeding the engine. He started a shallow bank to the left and feels the wind may have picked up the right wing and the aircraft stalled. Witnesses stated the aircraft was removed that evening and placed back into the trailer it was brought in to the airport earlier that day. The inspectors found the aircraft in winston-salem at the previous owner's (and builder's) residence. The piece of propeller from the scene matched up to the remaining propeller on the aircraft. The aircraft appears to be destroyed. The pilot was reported to have had minor injuries but he denied they came from the accident.
