From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Helicopter came in on an initial landing; the pilot said that it was sloppy so he picked it back up (take-off). The helicopter shot straight up to about 30-40 feet and completing 1 ½ -2 rapid turns. It then came down at a sharp nose down attitude come close to level before impacting the ground. The helicopter impacted the ground on the nose to the left side and then rolling over to the left. The tail rotor gear box and blades departed the helicopter. Main blades were damaged but stayed with the helicopter main rotor head.
