From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

On oct. 5, 2012, N6661P, Piper PA-24-250, departed cahokia, IL (kcps) at aprox. 1055 CDT enroute to meadowlake airport (kfly) Peyton, CO. On a IFR flight plan. The METAR weather at the destination at the time of arrival, 13006 kt 10 sm OVC018 04/M01 A3019. Enroute, the pilot encountered icing conditions. Near hugo VOR, 45.1 NM from meadowlake, the pilot noticed a substantial amount of ice building up on the aircraft (he was at 9000 MSL). He asked for lower altitude and was given 8500 MSL. About 10 miles from the final destination, he was VMC and had the airport in sight. He cancelled his IFR flight plan. Knowing he had a buildup of ice, the pilot maintained a faster approach speed than usual. At aprox 6-8 feet AGL, the pilot flared and reduced the throttle. The aircraft nose dropped and impacted the ground. The aircraft came to rest aprox. 150-200 feet after initial impact. The aircraft was destroyed.
