From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

09-11-13, The PIC and 1 passenger (pax) departed lake hood strip (Z41) in anchorage (anc), AK, bound for iliamna (ili). Acft was fueled in anc and topped off with appx 3 ½ hours of fuel. A pre-flight insp was not conducted prior to dep. The PIC received a wx brief and filed a flight plan with kenai (ena) flight service station. Wx was VFR conditions for dep, enrt and destination. At ili, PIC fueled acft again, topped off appx 10 gal of fuel.

At dep ili, no flight plan filed or wx brief received. A pre-flight insp was not conducted prior to dep. Wx was still VFR onditions. Wind direction was appx 250°, wind speed was negligible. Flight was direct ili to the head of knw river, a remote strip. The PIC landed at knw remote strip without incident. PIC and pax remained overnight at camp site.

On 09-12-13, PIC and pax took-off acft and flew area of knw river assessing the river braids and region. A pre-flight insp was not conducted prior to dep. Wind was on nose of acft, approximately 20-25 mph. Prior to landing again at knw remote strip, there was a decreased wind speed. Upon landing, acft hit a hummock (mound) that was appx 6? High about a 30° upslope, a little right of the center of the runway strip. The tire blew, rolled partly off rim. The wheel dug a trough approximately 3-4? Deep. The prop hit the trough and the left wingtip struck the ground after rotating over the gear. PIC shut down the master cut off switch and checked on pax. Both deplaned immediately and assessed damage to acft.
