From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

On 10/21/2013 the teb FSDO received a call that N8V, an experimental Lancair IV P, had a gear problem and was off the left of runway 24. According to the pilot, identified as privacy data omitted, private pilot certificate #privacy data omitted, the aircraft was on approach into Teterboro when the left main landing gear indication did not illuminate. privacy data omitted Did a low pass and asked the tower to verify if the gear was down. The tower stated that it appeared there were three wheels down. privacy data omitted Then departed the area and manually tried to extend the gear. The indicator would still not illuminate. privacy data omitted Returned to the airport and set the aircraft down on the centerline of the runway. The left main collapsed at this time causing the aircraft to drag the left wing on the ground and depart the runway. The aircraft sustained damage to the left wing tip. Teb tower verified the aircraft did call in with gear problems. The aircraft was raised and the gear was extended, it was then towed to the ramp with a ratchet strap holding the gear in the down position. privacy data omitted Has contacted the FSDO and claimed the cause of the gear collapse was a faulty left main gear actuator. In this aircraft the actuator pulls the gear into the down position. The aircraft's records were requested however, have not been made available for review by the FSDO as of this time.

As the FAA filed it

ON 10/21/2013 THE TEB FSDO RECEIVED A CALL THAT N8V, AN EXPERIMENTAL LANCAIR IV P, HAD A GEAR PROBLEM AND WAS OFF THE LEFT OF RUNWAY 24. ACCORDING TO THE PILOT, IDENTIFIED AS ^PRIVACY DATA OMITTED^, PRIVATE PILOT CERTIFICATE #^PRIVACY DATA OMITTED^, THE AIRCRAFT WAS ON APPROACH INTO TETERBORO WHEN THE LEFT MAIN LANDING GEAR INDICATION DID NOT ILLUMINATE. ^PRIVACY DATA OMITTED^ DID A LOW PASS AND ASKED THE TOWER TO VERIFY IF THE GEAR WAS DOWN. THE TOWER STATED THAT IT APPEARED THERE WERE THREE WHEELS DOWN. ^PRIVACY DATA OMITTED^ THEN DEPARTED THE AREA AND MANUALLY TRIED TO EXTEND THE GEAR. THE INDICATOR WOULD STILL NOT ILLUMINATE. ^PRIVACY DATA OMITTED^ RETURNED TO THE AIRPORT AND SET THE AIRCRAFT DOWN ON THE CENTERLINE OF THE RUNWAY. ^PRIVACY DATA OMITTED^ HAS CONTACTED THE FSDO AND CLAIMED THE CAUSE OF THE GEAR COLLAPSE WAS A FAULTY LEFT MAIN GEAR ACTUATOR. IN THIS AIRCRAFT THE ACTUATOR PULLS THE GEAR INTO THE DOWN POSITION. THE AIRCRAFT'S RECORDS WERE REQUESTED HOWEVER, HAVE NOT BEEN MADE AVAILABLE FOR REVIEW BY THE FSDO AS OF THIS TIME.
