From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

At 0900 on November 6, 2013, the great lakes regional operations center notified the Chicago flight standards district office (ord FSDO) that republic airlines flight number 4332, an ERJ-170-200LR, N407YX, slid off the side of runway 27L after landing and came to rest between taxiways H1 and A1. The reported weather at the time of the incident was 1452Z 20017G22 7SM ra br BKN010 OVC014 14/12 A2982.

At 0945 inspectors from the ord FSDO arrived on the scene to find the subject aircraft slightly off the south edge of runway 27L with its nose and left main landing gear in the grass. The fire department, which had responded within minutes, was still on the scene standing by. The passengers were already transported to the terminal but the flight and cabin crew remained on-board. The flight crew was briefly interviewed and their information was obtained. No specific information was provided by the flight crew explaining how this event occurred. The cabin crew was also interviewed and confirmed there were no passenger injuries.

The runway was examined and found to be wet but no standing water. The aircraft left the center line of runway 27L, 581 feet where it finally rested between taxiways H1 and A1. Evidence showed that at least one runway light was damaged by the right main landing gear. Another runway light was also damaged but may have been a result of emergency vehicles.

After calling the NTSB, the aircraft was released and towed to the hangar for an additional inspection. At that time, no apparent damage was visible other than a small cut in the number 3 tire.

Republic airlines maintenance personal forwarded their report to the ord FSDO on November 8, 2013 describing their inspections and actions. Their inspection necessitated the removal and replacement of the number 1 and number 2 main landing gear (MLG) wheel assemblies, number 3 MLG tire, nose landing gear tire assembly and the number 1 engine air inlet.

During this investigation the certificate management unit (cmt) for republic airlines provided additional information concerning this incident as provided by the FDR. No aircraft anomalies were found and the aircraft was in an airworthy condition. Both flight crew members filed asap reports and were trained in accordance with their asap mou.

Further actions or recommendations will be accomplished by the republic cmt.
