From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Aircraft left Bombardier Ft Lauderdale facility after troubleshooting flap problem to perform test flight. After takeoff and climbing through 800 ft plane yawed right and airframe vibrated slightly. Co-pilot verbalized right engine fail, lowed nose and leveled off at 2000FT. Crew contacted ATC ,declared an emergency and landed safely back at Ft Lauderdale international. FSDO-19 personnel inspected the aircraft and observed major damage to the right engine HP turbine blades as viewed from the rear of the engine. Also identified a few holes on airframe strakes located at tail section as a result of damaged turbine blades exiting engine.

Found out that the operator retrix aviation inc (6RXA) had just performed a conformity inspection on the aircraft to add it to its certificate. Also found out that standard aero(AN3R) out of los angeles performed a czi & mpi inspection on the engine 37 hrs prior to it malfunctioning. Reviewed right engine digital electronic engine control(deec) records with standard aero local reps which showed no abnormalities. Engine was transported to standard aero los angeles facility for investigation . An investigation was performed with los angeles fsdo- WP23 personnel present. Received email from FSDO-WP23 personnel stating the following? The only immediate conclusion from Honeywell so FAR is they saw nothing deficient in the build of the engine. They will be sending some blades off for testing to see if the engine had been overtemped, and will be continuing their investigation to root cause. Waiting on final engine teardown report from operator.

Received email that Honeywell issued alert bulletins on 6/5/2014 for LPT2 blade separation on the TFE731 engines.

Followed by an emergency AD 2014-12-52 issued on 6/10/2014. Honeywell is looking into whether or not the AD pertains to the engines on N113RX. Waiting on final teardown report.
