From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Parachutist, arkady shenker, age 49, departed aircraft N166DH, a Dehavilland DHC-6-100 on his 388 jump at 13,250 feet AGL. Type of parachute used was a SABRE2 manufactured by performance design, p/n sh 020P03, s/n SA-210-013230, and manufactured in 04/2011.

Harness assembly was a mirage dual container system, p/n: 64 M6, s/n: 9784

Date of manufacture: 20 April 2011. Jumper was wearing a phoenix ? Fly s/n 2699 flying suit. The following jump information was obtained from the parachutist personal alti-2 altimaster N3, s/n N130740, an audible altitude alert/log device mounted in jumpers helmet and a helmet mounted video cam recorder (contour roam, model 1600). Exit altitude was 13,250 feet AGL on 20 April 2014, at 14:18 local, free fall was 66 seconds, canopy time was 153 seconds. Main canopy deployed normally at 5300 feet AGL. Slider was stowed at 196 seconds, 9 seconds later the toggle handles were pulled down to release the stowed steering/brake lines. The left toggle released fully, the right toggle did not release fully and the parachutist began a right turning spin. Numerous attempts were made to fully release the right stowed toggle steering/brake line. The jumper grab the right toggle handle and steering/brake lines attempting to obtain a full release but this caused the finger trap loop to bind to the toggle handle tongue. The video indicated that no attempt was made to execute emergency procedures by releasing/cutting away the main canopy and deploying the reserve parachute.

A helmet mounted video cam recorder (contour roam, model 1600) recorded the jump from exiting the aircraft until impact, at approximately 14: 22.23 local.

The reserve chute was found to be overdue the required 180 day repack inspection, i/a/w 14 CFR part 105.43 (b) (1).

As the FAA filed it
