From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

UA206 from den (757, ac #N529UA) was being marshaled into gate #12A by a tdy ua ramp supervisor from ewr. A dgs agent trained to operate the jet bridge was in the cab. The jet bridge was parked entirely outside the safety envelope and its wheels were inside the stow box, however it was not raised to the level of a 757 door. An unauthorized stop mark was painted on the ramp to indicate the supposed stop mark for the L2 door of the 757. The ac was marshaled to that stop mark and the #1 engine struck the plb. Video indicates that several people on the ramp were attempting to get the marshaler's attention to stop the aircraft.

Findings:

1. Gate A12 ? Standard layout not approved for B757

2. Incorrect temporary markings for B757

? Original markings were placed on gate A12 during a kroger charter event in April 2013

? A B757 was placed on the gate as a guide for the temporary markings in 2013. Jetbridge position is unknown for the fit test

? According to the kroger (2013) charter schedule, both 757-200 and 300 series where operated. Gate A12 was not scheduled ? A11 was planned for 757 and remote cargo ramp was planned for 747

? It is unknown if temporary markings were placed with a 757-200 or 300

? Markings were refreshed by dgs this year for the derby

3. Jetbridge was parked outside the safety area. No stow box for mainline aircraft

4. Cre 47-5 process for gate layout variances and/or alterations not followed

5. Arrival procedure

? Last minute gate change from A11 to A12

? Temporary marking labeled ?57? Was not used direction was given to use the unmarked line approx.10FT away from the ?57? Line to accommodate the jetbridge for the L2 door

? Ramp tdy supervisor confirmed with dgs that the unmarked line was to be used

? Jetbridge was pre-positioned by dgs agent with guidance from ramp supervisor

? Ramp equipment was parked outside the safety area

? Left wingwalker signaled the marshaller to stop the aircraft just prior to contact with the jetbridge

VIDEO INDICATES THAT SEVERAL PEOPLE ON THE RAMP WERE ATTEMPTING TO GET THE MARSHALER'S ATTENTION TO STOP THE AIRCRAFT.
