From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

A witness watched the entire accident sequence and stated, ?the aircraft gained groundspeed and I observed the tail come up, eventually to a more than level attitude. The nose was pointing slightly toward the ground. The pilot corrected and the tail wheel impacted the ground. Immediately the aircraft became airborne, staying in ground effect. The left wing drooped slightly and then the aircraft began to turn (pivot) to the left on its axis. It continued moving east of the runway as it was traveling south bound. At about 500 feet from the takeoff point, the left wing dropped dramatically and the aircraft impacted the ground. It continued to spin, doing a 180 (degree turn) and finally came to a stop as the tail impacted the hangar door that faces west toward the runway.? The photographic evidence collected by this investigator supports this witness account.

As the FAA filed it
