From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

On 06/07/2014 on hwy 14/16, north east of Arvada WY, a bht 407 helicopter, N189AM, operated by air methods corporation, had an empty ambulance gurney roll into the moving tail rotor assembly. The helicopter had landed perpendicular and on the center of the road (crown). No one retained positive control of the gurney during the patient transfer from the gurney to the helicopter. Subsequently, the gurney rolled towards the edge of the road and struck the tail rotor. The gurney was sent into the ditch. Pieces of the damaged tail rotor blades and gurney traveled forward the left side of the aircraft damaging the tail boom, elevator and fuselage and striking one of the emergency medical responders (EMS) ground ambulance personnel.

The injured EMS responder was hit in the right abdominal area which caused bruising. The responder was evaluated by the local hospital, and held overnight night (less then 24 hrs) for observation as a precautionary measure and released the next morning.
