From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Description of events by privacy data omitted; certificate no.privacy data omitted:

On the fourth flight of the day in aircraft N1936T (PA28R-200), privacy data omitted departed C56, bult field airport in monee, Illinois, enroute to kpjc, Zelienople Municipal Airport in Zelienople, Pennsylvania. During the rotation phase of the takeoff roll, privacy data omitted described a ?shimmy? Effect, presumably coming from the landing gear. The landing gear was retracted normally; however, the gear-in-transit light remained illuminated, indicating the gear was not properly locked in the up position. After assessing the situation, privacy data omitted decided to continue to the destination. The enroute portion of the flight, conducted under visual flight rules, was described as ?uneventful?. Upon reaching kjpc, privacy data omitted lowered the landing gear which resulted in the nose wheel and left main landing gear down indication lights to illuminate green; in other words, successfully down and locked. The right main landing gear did not illuminate green. privacy data omitted Recycled the landing gear in an attempt to successfully extend all three landing gear, but was unable. privacy data omitted Departed the traffic pattern to the north and completed the aircraft emergency gear extension checklist and several maneuvers, which also proved unsuccessful. Another aircraft departed and flew past privacy data omitted and confirmed the right main landing gear appeared not to be properly extended, but that it had extended slightly out of the wheel-well.

Once determining an emergency landing was inevitable, privacy data omitted discussed different options with the aircraft?s mechanic via CTAF frequency. He decided the safest option would be to land with the gear retracted and began to burn fuel in the event of a fire on landing. privacy data omitted Landed the aircraft, causing only minor damage and himself and the sole passenger onboard exited the aircraft without injury. The aircraft was hoisted onto a trailer and moved to a hangar facility. After investigation of this incident, it was determined that the right main landing gear torque link center bolt was missing.

I cannot ascertain whether the bolt/castleated nut combination failed or was properly secured as the bolt and nut were not available for examination. Damage consisted of minor abrasion to the lower cowl, the two longitudinal rails at the bottom of the fuselage, inboard trailing edge flap abrasion, abrasion to the wing walk assist step and static port assembly. The engine experienced a sudden stopage and the propeller was curled back approximately four inches from the tip inward towards the hub.

DESCRIPTION OF EVENTS BY ^PRIVACY DATA OMITTED^; CERTIFICATE NO.^PRIVACY DATA OMITTED^:

ON THE FOURTH FLIGHT OF THE DAY IN AIRCRAFT N1936T (PA28R-200), ^PRIVACY DATA OMITTED^ DEPARTED C56, BULT FIELD AIRPORT IN MONEE, ILLINOIS, ENROUTE TO KPJC, ZELIENOPLE MUNICIPAL AIRPORT IN ZELIENOPLE, PENNSYLVANIA. DURING THE ROTATION PHASE OF THE TAKEOFF ROLL, ^PRIVACY DATA OMITTED^ DESCRIBED A ?SHIMMY? EFFECT, PRESUMABLY COMING FROM THE LANDING GEAR. THE LANDING GEAR WAS RETRACTED NORMALLY; HOWEVER, THE GEAR-IN-TRANSIT LIGHT REMAINED ILLUMINATED, INDICATING THE GEAR WAS NOT PROPERLY LOCKED IN THE UP POSITION. AFTER ASSESSING THE SITUATION, ^PRIVACY DATA OMITTED^ DECIDED TO CONTINUE TO THE DESTINATION. THE ENROUTE PORTION OF THE FLIGHT, CONDUCTED UNDER VISUAL FLIGHT RULES, WAS DESCRIBED AS ?UNEVENTFUL?. UPON REACHING KJPC, ^PRIVACY DATA OMITTED^ LOWERED THE LANDING GEAR WHICH RESULTED IN THE NOSE WHEEL AND LEFT MAIN LANDING GEAR DOWN INDICATION LIGHTS TO ILLUMINATE GREEN; IN OTHER WORDS, SUCCESSFULLY DOWN AND LOCKED. THE RIGHT MAIN LANDING GEAR DID NOT ILLUMINATE GREEN. ^PRIVACY DATA OMITTED^ RECYCLED THE LANDING GEAR IN AN ATTEMPT TO SUCCESSFULLY EXTEND ALL THREE LANDING GEAR, BUT WAS UNABLE. ^PRIVACY DATA OMITTED^ DEPARTED THE TRAFFIC PATTERN TO THE NORTH AND COMPLETED THE AIRCRAFT EMERGENCY GEAR EXTENSION CHECKLIST AND SEVERAL MANEUVERS, WHICH ALSO PROVED UNSUCCESSFUL. ANOTHER AIRCRAFT DEPARTED AND FLEW PAST ^PRIVACY DATA OMITTED^ AND CONFIRMED THE RIGHT MAIN LANDING GEAR APPEARED NOT TO BE PROPERLY EXTENDED, BUT THAT IT HAD EXTENDED SLIGHTLY OUT OF THE WHEEL-WELL.

ONCE DETERMINING AN EMERGENCY LANDING WAS INEVITABLE, ^PRIVACY DATA OMITTED^ DISCUSSED DIFFERENT OPTIONS WITH THE AIRCRAFT?S MECHANIC VIA CTAF FREQUENCY. HE DECIDED THE SAFEST OPTION WOULD BE TO LAND WITH THE GEAR RETRACTED AND BEGAN TO BURN FUEL IN THE EVENT OF A FIRE ON LANDING. ^PRIVACY DATA OMITTED^ LANDED THE AIRCRAFT, CAUSING ONLY MINOR DAMAGE AND HIMSELF AND THE SOLE PASSENGER ONBOARD EXITED THE AIRCRAFT WITHOUT INJURY.
