11/25/2014 ? Received phone call from poi maverick helicopters, reference helicopter, N876MH an EC-130-T2 model helicopter, operated by maverick helicopters (M7KA) that had ingested a sun screen through the tail boom fenestron, while on final approach into the Grand Canyon. As the pilot descended into the Grand Canyon the baggage door opened and a sun screen exited the baggage compartment; which then entered the fenestron; which is attached to the tail boom of the helicopter; which became shredded and caused the fenestron blades to become damaged and causing the tail rotor drive shaft to have a limited ?sudden stoppage?; which was indicated by the tail drive shaft fasteners indicating a crack at specific attaching points.
