From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

At approximately 13:20 the aircraft departed kbtv (burlington VT) with CFI privacy data omitted and student pilot privacy data omitted on board for a training flight. They flew to kpbg (plattsburgh NY) where they performed 3 cross wind landings. They departed kpbg heading approximately NE, as they approached the kbtv area by lake champlain the CFI pulled the throttle to idle and asked the student to demonstrate emergency landing procedures. The student located and set up for a landing in a field. The CFI suggested that they could land on lake champlain. The CFI told the student that he had landed on the lake the day before April 10, 2015. The aircraft was then set up for landing and landed on the lake. After touch down and after approximately a 100FT roll out, the nose wheel broke through the ice, and then the main wheels broke through the ice. Both the CFI and the student were able to get out of the aircraft and then walk to shore.
