From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

Observations

Overall impression; all equipment appears correct and in good working order. Both main and reserve parachutes were deployed and are correct. The reserve ripcord was found at the impact site less than 10 feet from Mr. Lapointe. The reserve deployment device was brought to the scene and I was able to do a quick inspection and found no discrepancies. I have not seen it since.

The harness was cut at the leg straps and the chest strap unthreaded at the friction bar. The cut-a-way pillow on the right side of the harness was in the correct location and was not activated. Both three ring canopy release assemblies for the main parachute were intact and operated correctly when operated at the end of my inspection. The velcro on both sides of the main lift web for the reserve ripcord and cut-a-way are in correct working order. No defects of the main parachute container is noted. The bottom of the container main pilot chute pouch has a small 1? Cut. The closing loop of the reserve container is intact and not cut by the cypres automatic activation device. The cypres has been removed and sent to ssk, the us manufacturer?s representative for inspection and possible data download. The packing data card (maintenance record) is with the equipment, correct, complete and indicated the parachute assembly was inspected and packed on 1 February 2015, well within the 180 day requirement prescribed by the federal aviation administration. All hardware is correct and functioning. The left reserve riser group, that connects the reserve parachute to the harness, was cut. Presumably by the first responders and is consistent with their procedures.

The reserve parachute is fully deployed, the slider is in its proper position, just above the connector links and all lines are straight. The deployment brakes are still stowed. Inspection of this parachute revealed a system that is in very good condition.

The main parachute was fully deployed and correct as well. The ?bungee? Style main pilot chute is in correct working order. The main deployment bag is correct, no broken rubber bands and all grommets are good. The main parachute has several patches, all well done and no functional issues are noted. The patches are consistent with a parachute of the 1999 vintage. The deployment slider is found at the connector links as expected. The collapsible lanyard options were not used. The right line group is cut in several places, again consistent with first responder actions. The main deployment brakes were cleared and both control lines appear to be functional.
