From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(1) The reason for the evacuation:

A. air cycle machine malfunction, which introduced smoke/haze into the cabin.

(2) Who initiated the evacuation:

A. the pilot in command

(3) Which exits (by specific location) were used, which exits were not used, and reason for nonuse:

A. escape slides were deployed at all eight exits. The escape slide at door 1L was not used for evacuation due to the fact that the slide was initially twisted by the gusty winds and was determined to be usable by the flight attendant manning that exit.

(4) Whether any exits, slides, or associated components malfunctioned and, if so, what were the malfunctions:

A. the escape slide at door 1L was not used for evacuation due to the fact that the slide was initially twisted by the gusty winds and was determined to be usable by the flight attendant manning that exit. The slide eventually righted itself but was not used for the evacuation.

(5) Whether any particular exit was used by the majority of the evacuees and, if so, the apparent reason:

A. the exits at doors 3 l & r appeared to be used by the majority of the evacuees. This may be attributed to there being more passengers seated or congregating in that area of the cabin.

(6) The estimated total time it took to evacuate the aircraft:

A. approximately 1 & ½ minutes.

(7) The nature and extent of any injuries:

A. two passengers were taken to maui hospital for possible ankle injury and tail bone bruising; both passengers were subsequently released later that afternoon and cleared to travel onwards.

--------------------

Flight 24 ogg - oak departed ogg. Passing through 7,000 feet on initial climb flight attendant called flight deck to report mist, acrid smell, fumes/odor in cabin. Captain relayed message to ATC, declared an emergency and initiated air turn back to ogg. After an uneventful landing in ogg aircraft came to a complete stop to assess the situation. The fire department performed an outside inspection of the aircraft and informed the crew there was no evidence of smoke/fumes/fire. The crew determined the inside and outside of the aircraft to be "all clear". The crew proceeded to taxi the aircraft off the runway to gate 1. However, after clearing the taxi way the flight attendants reported the situation was worsening. The captain made the decision to stop the aircraft and ordered an immediate emergency evacuation. Emergency exits opened and slides deployed and inflated; passengers and crew evacuated aircraft.

As the FAA filed it
