From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Witness statement: " aircraft taxied to north end of rnwy 17 . Heard aircraft engine go to to full throttle. Aircraft proceeded down rnwy until approximately 400-500 ft and then veered off runway approximately 45 degree angle. Aircraft proceeded on this track crossing open area, closed diagonal runway, taxi way and into a drainage ditch proximately 1500 feet from runway. It did not appear pilot input any control in the aircraft after pushing full throttle in on takeoff roll. Aircraft veered off runway with no indication of braking, steering, or attempt to liftoff. Aircraft rolled approximately 1500 ft after departing runway. Engine was still at full throttle when aircraft dropped into drainage ditch and impacted opposite bank of ditch."

Pilot sustained serious injuries.

Pilot made statement (post accident) 8/7/2015, that he had flown the aircraft around the airport , 2 separate trips in the pattern prior to the accident. On the next planned flight he had sat in the aircraft while it was being hand prop started for a somewhat lengthy period of time. The heat index was 105 degrees f.

After starting the aircraft , he taxied out to the end of runway. He stated he remembering pushing the throttle in and then everything going black.

He stated he had no further recollection of the events after that. He believes he lost consciousness due to heat exhaustion. He does not know if he was diagnosed or received treatment for heat related stress.

As the FAA filed it
