The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.
On 8/8/2015, at 1200 EDT N5491J, a Cessna 172S owned by aero safety training ltd had an incident while landing on runway 19 at the Lincoln Park Airport when conducting a training flight. VMC prevailed, no flight plan was filed. The aircraft sustained minor damage and the crew was not injured. The instructor pilot holds a CFI and commercial certificates the student pilot holds a student pilot certificate. The flight originated at Lincoln Park Airport on 8/8/2015 at 1130.
On takeoff the student failed to maintain proper directional control of the aircraft which contacted a runway light with the left main landing gear, causing damage to and disabling the left brake. The instructor took control of the aircraft and completed a normal takeoff. The pilots were unaware of the damage and continued into the pattern. Upon landing roll out, after the brakes where applied, the left brake failed and the aircraft exited the runway to the right. The aircraft came to a stop after impacting a row of bushes. The iic arrived on 8/10/2015 and completed an inspection of aircraft. Visual damage was determined to be a broken bleed fitting on the left brake. The broken bleed fitting allowed all of the hydraulic fluid to leak from the left brake causing it to fail. Witness marks on the propeller and bushes indicated the propeller was turning on impact with the bushes. An airman reexamination under 49 usc § 44709 will be scheduled. If additional information comes available this report will be amended as necessary.
