From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

On September 12, 2015 at 0210Z, a Gulfstream G-IV aircraft, registration number N615RD and serial number 1006 experienced damaged to the number 3 and 4 main wheel assembly during takeoff from westhampton (gabreski) airport (kfor). During taxi and after the takeoff, the passengers reported hearing two ?bangs? To the flight crew. The flight crew performed a fly-by to the tower at kfok, and they reported that the number 4 main wheel assemblies were not present on the aircraft and the number 3 wheel assembly was damaged.

The aircraft flew for three and a half hours over kfok airport to burn off fuel, and it was diverted to Stewart international airport (kswf) by new york center air traffic control for an emergency landing. At approximately 0600Z the aircraft landed at kswf airport with no injuries to the crew or passengers.

Gulfstream service engineering department inspected the aircraft and performed an evaluation of the damage to the aircraft. They found the following during the damage assessment:

? The RH MLG assembly exhibited staining, scuffing, and discoloration

? The RH MLG was compressed with chrome showing

? The RH tires and wheels were damaged

? The RH brakes were ground away.

? The hydraulic lines and wire bundles attached to the RH MLG were damaged

? The RH MLG wheel well exhibited multiple signs of FOD impacts including debris, scuff marks, bent and cracked tubing, broken wiring

? The RH MLG side brace exhibited scuffing from FOD impacts

? The flap drive rod exhibited a dented area.

? The outboard pillow block appeared to be out of alignment from the dented area in the flap drive rod

? The MLG door seal retainers were deformed and the seals torn at multiple locations

The damaged main wheel assemblies, wheel and tires debris was collected for the incident and sent to Gulfstream Aerospace corporation lab in savannah, Georgia to perform failure analysis of the damaged components.
