From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Helicopter, N290R, departed a 137 base in Forest Grove, Oregon at approximately 08:04 local time to ferry the helicopter to a field about 1 mile to the north. According to the pilot, the helicopter climbed to approximately 50 feet and 40 knots. Pilot stated the helicopter began losing lift. The pilot stated she pulled up on the collective to try to stop the descent. The pilot stated the helicopter continued to lose lift and impacted a row of berries. The helicopter was substantially damaged. The pilot received minor injuries.

The pilot was not wearing a helmet nor any type of hearing protection. The pilot stated she puts on the helmet and mask prior to spraying but for ferrying the aircraft, she does not wear the helmet so she has better peripheral vision for the crew.

Waiting as of 10-06-15 for helicopter manufacturer representatives for mechanical investigation of aircraft.
