From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

On 12-17-2015 @ 0915 N929SS, s/n RB-15 departed runway 8 kphx and declared an emergency stating he hit a bird and requested to land.

The aircraft landed back on kphx runway 8 and taxiied back to the company hangar on the north side of the airport.

Kphx airside operations briefly closed runway 8 due to FOD from N929SS being deposited in various spots. An American airlines flight crew states on tower tape that they hit FOD. A second American airlines aircraft that had already declared an inbound medical emergency was being directed to another runway due to the potential FOD per tower tape.

Airside operations retrieved the debris and gave it to the crew of N929SS.

The #1 engine had debris from the left nose cargo compartment ingested into the engine per information from a duncan aviation tech performing a borescope on the engine.

The #1 engine was removed and a loaner engine was installed as viewed in the engine log book, the primary engine was sent to williams for inspection and repair.

Interviews with the PIC, SIC, dom and aircraft owner were not conducted due to all being represented by counsel who advised his clients not to speak with the sdl FSDO.

The facts are the fwd l/h cargo comparment was not secured prior to t/o, the aircraft deposited the contents of the cargo compartment onto runway 8 and into the #1 engine. The #1 engine was removed for inspection and repair.
