From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Block 33: this office (GL31) was notified 29 dec 15, around 9:15 am (CST), by a representative of japan air lines (jal) that they had been notified the previous day that their flight, jal 9 kord ? Rjaa, had possibly lost parts on kord rwy 10L on 28 dec. Subsequently a swift air charter B-737-400 had aborted a takeoff due blown tires caused by FOD on the runway.

Kord ground operations conducted a FOD inspection after the aborted takeoff and retrieve a collection of parts later to be determined were from B-777 and B-737 aircraft.

FAA inspectors proceded to the jal line maintenance office below terminal 3 and were met by the all nippon airways manager. He explained that ana 11 (kord ? Rjaa, 28 dec) had also lost parts and that some of the retrieved part were very possible from the ana aircraft.

A summary of the parts found included several parts deemed to be from a B-777 main landing gear assembly, a part of a B-737 nextgen retractible landing light assembly, and part of an accoustic liner from an engine high-bypass fan shroud.

Summary:

1. Notified by jal at approx. 0915 (CST) 29 dec 2015 (via the r.o.c.).

2. ANA011 departed kord rwy 10L at 1504 (2104Z) for rjaa.

3. After takeoff, the flight crew received eicas messages ?auto speedbrake? And ?antiskid? (Consistent with the damage to the MLG junction box).

4. In cruise, the flight crew received eicas messages ?tire pressure, auto speedbrake, antiskid, gear indication sys, brake temp sys? (All consistent with the damage to the MLG junction box).

5. Kord had winter weather conditions for at least 8 ½ hours prior to the ana 011 departure with a mixture of rain, freezing rain, snow, ice pellets, and mist reported in the field METAR.

6. No evidence of any runway cleaning (snow removal attempts) prior to the ANA011 departure.

7. After arrival at rjaa (narita int?l), damage to the junction box on the right main landing gear.

8. At 1702 SWQ2536 aborted takeoff on kord due to blown tire.

9. Airfield operations closed 10L for FOD retrieval and snow removal at 1712. Aircraft debris was recovered from rwy 10L, between taxiways ?dd? And ?N1.?

10. The FSDO was notified until 16 hours after the aborted takeoff, limiting the inspector?s ability to ascertain the exact position of the debris on the runway and the condition of the runway.

As the FAA filed it
