From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

^Mr. Cosenza^ was giving flight instruction and transition training from a R44 helicopter to the 206 helicopter. The owner ^(Mr. Banks)^ had just purchased 1 month before (FAA records reflect it was purchased on 1/5/2016, and the registration certificate was issued on 2/3/2016). ^Mr. Banks^ and ^Mr. Cosenza^ performed all the preflight checks and flight control checks prior to the accident and had no anomalies as stated to the NTSB. ^Mr. Cosenza ^estimated they were lifting off from the dolly between 1235 and 1240 EST, and with his

^(Mr. Cosenza's)^ left hand on the collective during liftoff, the helicopter immediately rolled to the left impacting the ground and rolling one complete roll before settling on its side. The owner ^Mr. Banks^ broke his lower right leg in three area's and was taken to the hospital. A security video recording show's the aircraft performing the run-up and as it started to lift off the dolly, the nose of the helicopter started to turn to the right and next the helicopter was starting to roll on its left side. The helicopter was substantially damaged during the accident and during the investigation of the wreckage there was no mechanical failure that could be found.

A 3000 hour maintenance inspection was performed on December 21, 2015 and during the investigation there is no appearance of faulty maintenance.

^Mr. Banks^ gave a statement to the fact that during the transition of lifting off the dolly the aircraft quickly rolled on the left and has no idea what happened. ^Mr. Banks^ identified himself as the pilot in command (PIC).

As the FAA filed it
