From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

At Lodi Airport, CA. Parachute center, two skydivers using a u.p.t. tandem parachute rig was fatally injured from impact with the ground.

Tandem instructor was yonghyeon kwon and passenger was a tyler turner.

San joaquin sheriffs found tandem skydivers southwest of the Lodi Airport, secured the scene, identified the two men and forensic photographer took pictures & video.

The tandem skydivers had exited aircraft at approximately 13000 feet with no issues.

Kwon had extracted/thrown out main parachute white drogue.

The main chute drogue tether/bridle was out fully but the drogue itself did not fully deploy/inflate.

Kwon may have been confused by main drogue not inflated correctly, swaying back and forth.

Kwon does not release/deploy main parachute at any time.

Somewhere around 3000 feet, kwon pulls/releases the reserve/emergency parachute.

The reserve chute drogue hits the main chute drogue and very quickly the two different drogue tether/bridles start to wrap/twist on each other.

The reserve drogue continues rising above main drogue and inflates.

As the reserve chute is pulled further out of the parachute rig, its deployment bag goes up to the main white drogue.

At this point, the reserve chute knots up or tangles hard with main chute drogue.

This final knot /tangle prevents reserve/emergency chute from releasing fully/inflating.

Kwon releasing/deploying the reserve/emergency parachute instead of the main chute could be panic and/or lack of emergency procedure training.

Kwon did not have a uspa/u.p.t. tandem rating, had a total of 679 jumps & 68 tandem jumps, holds a australian parachute federation certificates/ratings a, b, c, d, e, star crest, packer b and holds a FAA third class medical dated 6/24/2016.

As the FAA filed it
