From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

On 08/16/2016 psa flight 5517, aircraft N216PS, phl-day, reported a birdstrike to the nose of the aircraft and the LH engine on approach into day. The aircraft landed safely and taxied to the gate. The next aircraft landing reported an engine cowling on the runway. No damage was reported by the landing aircraft. Upon inspection of the N216PS, it was discovered the lt engine upper cowl was missing from the aircraft and the lower nose access cowl was damaged. The upper cowl was recovered from the runway. Further inspection of the aircraft showed no evidence of a birdstrike on or in the left engine. A review of the logbook for N216PS shows that both the left and right engine cowls had been removed for maintenance the previous night. Further review shows that AD 2015-03-01 addressing this issue has yet to be complied with on this aircraft, however is still within the compliance time for completion. Evidence of improper installation could not be determined. The AD is currently scheduled for the next heavy check.
