From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Parachutist was injured on August 21, 2016, involving a parachute jump. He has 100+ jumps. This occurred at start skydiving in Middletown, Ohio. The skydiver had a pilot chute wrap around his leg on deployment of his main parachute. He then deployed his reserve parachute which open correctly.

He did not cutaway the main parachute, he pulled the main parachute to his body and held it between his legs until about one thousand feet above the ground. At that time the main parachute fell out and opened leaving him with two parachutes deployed. When the main parachute opened it caught his legs in the lines and caused him to hit the ground on his side.
