From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

1) Description of deviation: after a normal preflight and run-up the pilot started a takeoff run which was aborted at rotation when the pilot sensed a jam in the yoke. During the abort the aircraft over ran the runway, traveled across a turf field and came to rest on a perimeter road. The pilot made a hard right turn to avoid a drainage ditch and the left main landing gear folded up damaging the left wing . 2) causal and/or contributing factor not already addressed in blocks 13-16, if any: a windshield defrost/heated duct fell into the yoke linkage under the instrument panel restricting the aft motion of the yoke. Traveling over the rough turf field caused the duct to vibrate clear of the yoke linkage.

3) Recommendations for systemic corrective action to reduce risk of future occurrence: during annual inspections and when avionics are worked on there should be a visual inspection to confirm all wire bundles and ducting are securely tied and clear of the flight control movements.

4) Comments: none.

As the FAA filed it
