The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.
On may 31, 2018 at approximately 11:15 CST, an ayers, thrush model S2R-H80, serial number H80-180DC, N6215P registered to mid continent aircraft corp, operated by stokes flying service. During takeoff loss control striking small levy and sliding into a bean filed. The airman during takeoff lost his right rudder pedal due to the adjustment slide failing. The rudder pedal moved full forward contacting the hopper wall not allowing the airman to engage the right brake. Pulling the propeller into beta to slow the aircraft down, it veered left striking a levy and resting in the adjacent bean field. The propeller made contact with the ground, the left main gear and tail wheel collapsed causing major damage to the aircraft structure, and engine. No injuries reported.
