From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

On Wednesday, August 1, 2018, a Cessna TR-182, registered as N541BS, serial number R18201568, was involved in an incident at mcnary field airport (ksle) in Salem, Oregon. The incident resulted in minor damage to the aircraft.

The aircraft is registered to dalke debbie r, po box 668, aumsville, OR 97325.

The pilot explained that as he approached ksle, he extended the landing gear and could not get a positive indication that the gear was down and locked. He stated that he could see that the left and right mains were down but the indicator light would only flash green instead of indicating a steady green.

He cycled the landing gear a few times and then ran the emergency landing gear extension checklist which included pumping the landing gear down. After completing the checklists, a down land locked indication still could not be achieved. Pilot explained that he suspected an issue with the nose gear and attempted to land the aircraft as softly as possible to minimize damage. The following day maintenance personel had to physically pry the nose gear door open in order to get the nose gear to extend into the down position.
