What happened
The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.
On November 13, 2018, at approximately 1910, a disabled passenger in a wheelchair and a ramp service agent (rsa) fell from a baggage box that was being used to offload the passenger from the rear cargo door of a CE-208-B. A forklift was used to lift the baggage box. The passenger sustained minor injuries in the fall, the rsa was not injured.
The baggage box was not properly secured to the forklift, the weight in the box shifted and the box became unbalanced and fell towards the left wing of the aircraft and first struck the upper portion of the cargo door and then the left flap, causing damage to both. The box continued to fall, striking the ground on its left side, the occupants fell with the box. The rsa personnel called the ramp supervisor who assisted the passenger. The passenger and the rsa were sent to the Bethel hospital er.
To ensure this does not happen again hageland management put out an email to all station manger stating that if passengers can not load through normal doors then they will not fly. Also was briefed by the chief pilot to all flight crews at the monthly call-in meeting. This was posted and broadcasted throughout all stations.
