From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

On 05/31/2019 received notification from the roc of an event that occurred the previous evening. The pilot stated that at approximately 1930 CDT the pilot privacy data omitted of the above experimental amateur-built aircraft was on its initial test flight. The 30 minute flight was uneventful and without issue and the pilot executed a three point landing then ground looped on roll out. No injuries reported and minor damage to aircraft had occurred. Damage to aircraft was limited to r/h wing tip scrape, propeller tip damage and r/h landing gear.

Final investigation revealed weld failure at axle to flange area due to either filler rod composition, poor penetration and overall weldment design. The builder stated that he modified the axles/flange area to accommodate aluminum bolt on axles and oxy-acetylene welded. No issues noted with pilot or aircraft records and this matter is considered closed.

As the FAA filed it

ON 05/31/2019 RECEIVED NOTIFICATION FROM THE ROC OF AN EVENT THAT OCCURRED THE PREVIOUS EVENING. THE PILOT STATED THAT AT APPROXIMATELY 1930 CDT THE PILOT ^PRIVACY DATA OMITTED^ OF THE ABOVE EXPERIMENTAL AMATEUR-BUILT AIRCRAFT WAS ON ITS INITIAL TEST FLIGHT.
