During the flight, the pilot declared an emergency for loss of roll trim and landed safely at ict; the incident was almost catastrophic. Corrective action: on 10/15/2019 ^KG2R^ performed the following maintenance: discrepancy run away trim disposition- checked operation of trim. Found trim operational, but trimming in reverse. Checked continuity of all wiring. Found wires swapped at GP104J1 pins 3 and 9. Corrected as necessary. Attempted to perform functional check of system. Manual trim operates normal but no autotrim. Checked continuity of all wiring involved. Replaced questionable terminations in the radio junction box. Swapped gias removed and replaced 3931 K5 and K6, removed and replaced A194 pcb (roll card). Checked operation. System works properly. Tested system I.a.w. G5000 post installation check doc. 190-01508-N2 rev.4 system passes. Aircraft requires flight test before return to service. Incident investigation summary: 1.it appears from the evidence collected, that on 9/23/2019 ^bevan aviation (KG2R)^ installed STC ST01821WI on aircraft ^N116XP^, and returned it to service for the work performed prior to performing a flight test in accordance with gfc 700, TAWS, and post modification flight procedure steps 8 through 9. The aircraft was un-airworthy over a period of 5 months and operated by various pilots in that condition. The first flight performed after coming out of maintenance on 10/13/2019 was almost catastrophic. All flights performed after that were in violation of some of the following regulations 14 CFR CFR 91.503(a); 91.405(a); 91.405(b); 91.407(a)(1); 91.407(a)(2); and 91.407(b); root cause: lack of communication, lack of team work. 2.it also appears ^(KG2R)^ is in violation of the following regulation: 14 CFR 145.12(a)(1)(ii) repair station records: falsification, reproduction, alteration, or omission. Root cause: norms, pressure 3.further investigations revealed ^KG2R^, is in violation of 14 CFR 43.13(b) for not maintaining or altering aircraft ^N116XP^ in such a manner that the condition of the aircraft was equal to its original or properly altered condition. This was indicated by wires being swapped at GP104J1 pins 3 and 9 that almost caused a catastrophic accident due to a run-away trim condition according to the PIC of the aircraft. Root cause: lack of communication, pressure, lack of awareness, lack of team work. Investigation terminating actions: 1.on March 9, 2020 ^bancorp south bank^, the registered owner of aircraft ^N116XP^ ^(now N488B)^, s/n RK-427, had a flight test performed in accordance with STC ST01821WI post installation checkout procedure Beechcraft model 400A (ng, FJ44-4A) 190-01508-N2, steps 8.1 through 9. The findings of non-compliances and unsafe practices were discussed and referred to other FSDO safety inspectors for additional disposition as determined appropriate.
