From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Investigation results:

-On 11/17/2019, private pilot/owner of Navion, N5198K, ^Mr. Michael galbreath^ was in the process of completing a flight review with certified flight instructor, ^Mr. Michael johnson^.

-^Mr. Galbreath's^ flight review had expired, and he could not act as pilot-in-command for the flight. ^Mr. Michael johnson^ was the pilot-in-command during the flight review that he was conducting for ^Mr. Galbreath^.

-During an interview, ^Mr. Galbreath^ said that ^Mr. Johnson^ was talking to him about his performance on the first touch and go that was completed on rwy 25 at Lorain county airport (klpr), and he said that he became distracted on the downwind leg for the second landing on runway 25 at klpr, which resulted in a gear-up landing on runway 25 at klpr on 11/17/2019 at approximately 11:35 am EST.

-During an interview, ^Mr. Johnson^ stated that he was unaware that the landing gear was not lowered by ^Mr. Galbreath^ until after the aircraft touched down on runway 25, and they heard an unusual sound after landing with no rudder authority for steering the aircraft.

Root cause analysis:

-The gear-up landing resulted due to the following: distractions, lack of awareness, and complacency.

Corrective actions to be taken:

-Both ^Mr. Michael galbreath^ (private pilot/aircraft owner) and ^Mr. Michael johnson^ (certified flight instructor) have accepted remedial training offers from the cleveland FSDO. The cleveland FSDO will begin to develop a remedial training curriculum for each individual to regain safe aircraft operations. Once the remedial training is successfully completed by each individual, no further action should be required.
